Galway Bay fm newsroom - Almost 3000 mistakes have been made with patients' medication in the last 4 years within the Saolta Hospitals group - which includes UHG, Merlin Park and Portiuncula Hospital.
Official figures reveal that last year alone, the group, which includes hospitals in counties Galway, Mayo, Roscommon, Sligo and Donegal, recorded 834 incidents.
The figures relate to mistakes made by staff when administering drugs to patients.
Such errors are recorded with the health service's National Incident Management System.
According to data released under Freedom of Information to the Irish Daily Mail, over 15,000 incidents have been recorded across Ireland since 2013.
