The HSE has outlined its plans to close Toghermore House in Tuam.
The residential care facility on the outskirts of the town currently has 11 residents.
Over the past 5 years, the centre has been the subject of numerous campaigns amid fears of closure.
In response to a question posed by Tuam area councillor Donagh Killilea, the HSE says over the coming months, residents will be transferred into 'domestic orientated housing' in Tuam town.
It's in the process of buying three houses in Tuam town to house the 11 residents.
The transition will be carried out in partnership with the county council and Respond Housing Association.
Councillor Donagh Killilea says the closure is disappointing and counter-productive.
He says he intends to raise the matter at the HSE Regional Health forum meeting on Tuesday.
