The lowest number of COVID-19 deaths occurred in Leitrim at 26, with Longford having the same number of deaths as Galway city.
The figures incorporate all deaths where COVID-19 was identified as the underlying cause - by place of death, area of residence and age.
Dublin City recorded the highest number at 951, which represented 17% of all such deaths nationally.
59% occurred in general and orthopaedic hospitals, with 29% of deaths occurring in nursing homes.
While 91% of deaths were persons aged 65 and over.
