Galway Bay fm newsroom - Galway city and county councils have launched an initiative which aims to help older people to continue living in their homes.
The two councils will join seven other Local Authorities across the country to take part in the National Healthy Age Friendly Homes Programme.
The scheme, which is run in conjunction with Sláintecare and Age Friendly Ireland, aims to support older people by improving their living conditions and by offering access to continued health and social care supports as well as tailored community supports.
The two year programme has set a target of reaching 4,500 homes nationwide.
Organisers say participants have been identified as having significant social care needs or at risk of hospitalisation or premature entry into long term residential care.
Mark Harrington is Manager of the programme - he says the approach aims to support these people to live with a sense of independence and autonomy.
