Inspectors have found continued serious issues at a nursing home in Oranmore, despite some improvements.
Bushfield Care Centre has been the subject of three poor previous inspections - which identified serious issues in a range of areas, including staffing, food and finances.
During the latest inspection HIQA found the home had failed to pay a gas supplier on time - leaving residents without hot water for 24 hours, including showers.
Dining experiences, meal quality, food choices, activities, social engagement and outdoor access were all noted to be improved.
Interactions between staff and residents were seen to be friendly and respectful.
But there were some serious issues identified across a range of areas - some ongoing, and some new.
They included poor management, lack of progress on longstanding fire safety works, and issues around residents finances.
Inspectors were concerned about residents ability to access their own money - which was kept in the centre's operating bank account.
They also found some residents were charged for services they hadn't availed of.
Staffing levels were noted to be adequate, but some care staff were observed to be lacking in nursing knowledge.
Some weren't aware that some residents were at risk of malnutrition, or that some of them had wounds.
Overall, Bushfield Care Centre was found to be non-compliant with regulations in half of the 10 categories assessed.
